Transition from PCP to Community Health Leader
This session will review how the process can be beneficial to the facility and the community. As well as the outreach impact on patients and the new role as a health care provider.
Updates in Medical Weight Loss and Community Based Wellness
This topic is important for physicians because overweight/obesity is an epidemic, and it is co-morbid with several other chronic disease states we treat. There are more options for medical treatment than ever before, and because several of them are new, physicians may not be aware of them.
Exercise and Fitness after CVA
With the development of new medications and interventional procedures, patients are not dying from strokes, but rather living with disabilities. Some patients seem very motivated to prevent additional strokes and are eager to exercise.
Poppulation health as a model for healthcare delivery offers a promising opportunity for the integration of comprehensive medical fitness programs. This session reviews the evidence underlying medical fitness based lifestyle change programs, their integration within health systems, and the structure and process components to be considered for effective and efficient program design. We'll review the practical metrics in program evaluation, balancing the needs of the patient as a consumer and the demands of the hospital in attaining cost-effective outcomes. To fit these needs of population health integrated medical fitness programs, we'll review the emerging technologies that enable a scalable strategy.
