MFA Program Accreditations

Phase A – Application & Payment:

  1. Complete the online application below.
  2. Pay the application fee of $1,500 online at the time of application: https://www.medicalfitness.org/products/brain-health-program-accreditation
    * This fee includes a one-year MFA Facility Membership ($900 value). Current members may apply this to their next renewal.

Requirements for applying:

    • Open for business and operational for a minimum of 6 months prior to applying.
    • A minimum of 5 participants must have completed the program prior to applying. 
    • Hold an active MFA Facility Membership (included in the application fee).
    • Separate applications must be submitted for each additional site/location.

Phase B - Examination & Evaluation Report

  1. The Examiner will contact you to coordinate dates for the virtual live meetings.
  2. The Examiner will send you the Self-Scored Grading Checklist and instructions for all other requirements, which are due within 1 week of the first scheduled meeting.
  3. Following the submission of all files and the virtual live meeting with the Examiner, the Examiner will provide an evaluation report to the MFA office for review. Your score and final results will be communicated to you after that process is complete.
